Island Sexual Health Community Health Centre (ISH) is seeking nurse practitioners to work collectively with an interdisciplinary health care team at Island Sexual Health to provide a blend of longitudinal primary care services and focused sexual health services, along with services that address the social determinants of health to populations residing in Greater Victoria, Saanich, and the Western Communities. ISH is an existing Community Health Centre (CHC) that provides wrap-around sexual health care, peer-support, and sexual education. ISH CHC will be enhancing the centre’s existing sexual health services by extending its services to provide primary care. The nurse practitioner provides low barrier health care services as part of the interdisciplinary health care team in accordance with the Vision and Mission of Island Sexual Health Community Health Centre. The nurse practitioner is a contracted provider with ISH and will fulfill the obligations as outlined in the service contract including supporting patient attachment for primary care in accordance with Ministry of Health requirements for panel size and composition. The nurse practitioner will also provide episodic care for sexual health for both attached and unattached patients. The ISH patient population will consist of people 12 years and older, many of whom who face barriers to care such as histories of mental health challenges, problematic substance use, colonization, discrimination, trauma and poverty. Nurse Practitioner Duties: 1. Provide safe and effective longitudinal primary care and in accordance with the BC College of Nursing Professionals practice standards and guidelines. Patient population will consist of people with moderate to complex health care needs including structural vulnerability such a mental health and substance use challenges, poverty, discrimination, and colonization. 2. Primary care medical practice within the Nurse Practitioner’s scope of practice that includes conducting thorough medical histories and physical assessments, develop and implementing treatment plans, providing appropriate medical interventions, arranging specialist care and referrals as required, providing after hours call as part of the primary care provider group, and documenting in the electronic medical record as required. 3. Prescribes drugs within the statutory and regulatory standards, limits, and conditions for Nurse Practitioners. 4. Assessment and diagnosis of sexually transmitted infections. Competence in providing comprehensive sexual health services including independent testing and counselling for sexually transmitted and blood borne infections. Knowledge of prevention methods, including barriers, PEP, and PrEP. 5. Assessment, counselling and dispensing of birth control medications. Extensive knowledge about different birth control methods and medications including IUDs, combined hormonal contraception, progestin only contraception, and barriers. 6. Participating in professional development activities as required to provide effective and safe patient care. 7. Working collaboratively as part of the ISH interdisciplinary care team including consultation and shared care with team members. 8. Participate in interdisciplinary team planning for patient care. 9. Participate in quality improvement and quality assurance activities. 10. In collaboration with the ISH team, provide mentorship to a variety of learners including Nurse Practitioner students.
