Slhexun sun’ts’a’ Clinic provides full-service family medical care with an emphasis on health and wellness. The primary care teams work from an innovative First Nations driven model of care that is designed to meet the complex, holistic needs of our clients. Our team consists of a Family Practitioner, Nurse Practitioner, Registered Nurse, LPN/Health Coaches, Social Determinants of Health Worker, Dietitian/Certified Diabetic Educator, Chiropractor and Respiratory Therapist. Our clinic works closely with the departments at Ts’ewulhtun Health Centre which includes Kwun’atsustul Mental Health Counselling, Quw’utsun S’ulhween (Elders), Dental and Healthy Families Programs. The patient population that is served is primary Indigenous people who live in the Cowichan area. This includes those with complex care needs including chronic mental health or physical concerns. The NPs will be the most responsible primary care providers for a minimum complex patient panel as described in the NP Primary Care Service Contract Template. The NPs will participate as a member of an inter-professional team that includes Primary Care Providers (i.e. other NPs and FPs), Medical Office Assistants, Registered Nurses, and Social Workers, Health Coach/ Licensed Practical Nurse, Dietician, Certified Diabetic Educator, Chiropractor and Respiratory Therapist. Clinic Supplied Technology: - Electronic Medical Record, supplies, any necessary travel expenses Additional Sites: - Patient’s homes Preferred Experience: - Experience with Indigenous, medically complex patients experiencing mental health, comorbidity, and mobility challenges. - Experience using electronic health record (ideally MedAccess). - Willingness to participate in team-based care. Overhead: - The Ministry provides funding for NP overhead; the current available amount is $75,000 per FTE and will flow to Cowichan Tribes from Island Health.
