Cook Road Medcal Clinic is seeking a locum and a permanent full-time Family Physician to join our team on a full-time or part-time basis. This role involves providing longitudinal primary care to a diverse patient panel, supported by experienced MOAs and EMR integration, with flexible scheduling and physician cross-coverage. You will deliver comprehensive care to patients of all ages, building long-term relationships. The position offers a well-established patient base and a smooth transition into practice within a collaborative and supportive environment. Open to considering provisionally licensed family physicians for this role.

Open to considering the following practice arrangements to fill this family physician (FP) vacancy:
1. Permanent Full-time FPs
2. Permanent Part-time FPs
3. Locum-to-Permament FPs (FPs seeking to trial various practice settings prior to securing a clinical home & beginning to build a patient panel)
4. Career Locum, Permanent Float FPs (seeking roles that do not require building a patient panel - i.e., providing regular/scheduled walk-in, patient overflow or vacation coverage)

Minimum Licensure Required: Provisional license - i.e., open to sponsoring/supervising provisional registrants

Schedule flexibility: Open to considering partial or reduced hours.

EMR Expectations: Software training & support available on-site.

Payment Model Options:
- Longitudinal Family Physician (LFP)
- Fee-for-Service (FFS)
- Assignment of Payment (AOP)
- New to Practice (NTP) PCN Contract (for Return of Service (ROS) physicians)
