• Clinic Manager
• 2 Full-time Family Physicians
• 1 Part-time Family Physicians
• 1 Family Physicians handling walk-in patients
• 2 Full-time MOAs
• 1 Part-time MOA
• PCN Allied Care Professionals (3 Chronic Disease Management Registered Nurses; 1 Social Worker; 3 Clinical Counsellors; 1 Registered Dietitian; 2 Physiotherapists; 1 Occupational Therapist; 1 Clinical Pharmacist; 1 Frail Seniors Team Registered Nurse; 1 Frail Seniors Team Occupational Therapist; 1 Frail Seniors Team Social Worker; 1 Community Link Worker; 1 Administrative Coordinator; & Chronic Disease Group
