REACH Community Health Centre is a dedicated health hub committed to making a positive impact on comprehensive care. Our interdisciplinary team of professionals provides a range of health and social services, including a medical clinic, a dental clinic, a pharmacy, and an Urgent and Primary Care Centre. Complementing these healthcare teams, is our Health Equity and Engagement Department, strategically addressing the diverse social determinants of health.
They are looking to have a Nurse Practitioner join their clinical team. As part of a multi-disciplinary approach to full-service family medicine, an interested NP would work collaboratively in the team environment and start their own patient panel as per Ministry requirements.
This will be a full-time or part-time salaried position. This clinic is excited to add to their team a Nurse Practitioner interested in working together to provide primary care for the surrounding community of patients. This initiative is part of Primary Care Networks (PCNs) evolving in Vancouver. NPs interested in this opportunity should be willing to work collaboratively and participate in PCN development in the community.
Within REACH medical clinic, we offer a holistic approach to healthcare. Our team consists of seven Family Physicians (7 FTE) and two Nurse Practitioners (1.0FTE), ensuring personalized care for every patient. Supported by four nurses, two counsellors, and one social worker, we prioritize the well-being of our patients. Our dedicated team, is on-site daily for 9am to 5pm, benefiting from an approximate 1:1 ratio of MOAs to FPs. To ensure seamless operations, we have a full-time coordinator and part-time operations manager.
We take pride in offering active mentorship through peer group support and weekly meetings. Opportunities for 1:1 mentorship can be negotiated.
Our clinic is designed for both efficiency and comfort. Each physician enjoys a dedicated exam room per shift, and the clinic has a total of nine rooms. Communal areas, a group room, kitchen and outdoor patio provide spaces for team collaboration and relaxation. Free parking within two blocks of our Centre adds to the convenience.

– Work in collaborative interdisciplinary team with a wide range of health professionals: physicians, nurses, social work, counselling, dental, pharmacy, health promotion professionals and more.
– Provide responsive, patient-centered, holistic care for patients from a diverse cultural and socioeconomic backgrounds with a wide range of health needs.
What you bring:
– Current registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).
– 1 or more years of experience working as a Nurse Practitioner in a primary care setting.
