Vancouver Coastal Health is seeking an Intake and Transition Nurse (RN) to join the Primary Care team at Raven Song Community Health Centre in Downtown East Side (DTES), Vancouver BC. Apply today to join our team!
As a Nurse with the Primary Care Team/Integrated Care Team, you will:
Work as a member of an interdisciplinary team to conduct the intake and transition functions for the designated site/program services.
Provide clinical judgement and guidance for initial client screening, assess referrals and clients based on client presentation, urgency of needs and best approach to determine the appropriate intake treatment.
Coordinate discharge and transitions, as needed.
Support client transition by monitoring client connection to clinic and/or 3rd parties.
Assess and communicate clinic capacity to accept new referrals to health providers and the general public.
Establish and enhance relationships/communication within the designated program services and other community resources.
Collaboratively develop transition plans for new referrals, initiate and facilitate interdisciplinary care conferences, prepares and maintain client records, maintain and provide accurate intake/referral data and statistics, participate in team meetings and continuing education, and engage in on-going team planning and quality improvement.

QUALIFICATIONS
Education & Experience:
Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Two (2) years recent related experience in the designated clinical program area or an equivalent combination of education, training and experience.
Knowledge, Skills & Abilities:
Broad knowledge of nursing theory and practice within a client and family centered care model.
Broad knowledge of the British Columbia College of Nurses and Midwives (BCCNM) standards of practice for nursing.
Demonstrated ability to provide client-focused care in completing initial client assessments and interventions.
General of knowledge Trauma Informed Practice and Harm Reduction approaches and Strengths-based care.
Demonstrated ability to promote Indigenous Cultural Safety.
Demonstrated ability to provide competent and culturally safety care in a variety of settings and with diverse populations.
Knowledge of provincial acts, regulations, and program policies and guidelines related to home and community care.
Knowledge of community resources available for client/family support and related health services.
Broad knowledge of other health care disciplines and their role in client care.
Demonstrated ability to problem solve using sound judgement in applying critical thinking skills within safe limits of client care.
Demonstrated ability to communicate effectively, both orally and in writing.
Demonstrated listening and information seeking skills that promotes communication and lead to a co-operative approach to problem solving within a multidisciplinary setting.
Demonstrated ability to establish workload priorities.
Demonstrated ability to deal effectively with conflict situations.
Demonstrated ability to work both independently and collaboratively as a member of a multidisciplinary team.
Demonstrated ability to adjust to new or unexpected events.
Ability to operate related equipment.
Physical ability to perform the duties of the position.
Demonstrated computer literacy to operate a computerized client care information system and word processing, spreadsheet, Internet and e-mail software.
