The Vernon Urgent & Primary Care Centre (UPCC) is a team-based care clinic consisting of family physicians (FPs), nurse practitioners (NPs), primary care registered nurses (RNs), social workers (SWs), mental health counsellors, and a dedicated operational manager. The Vernon UPCC is a partnership between the Ministry of Health, Interior Health, the Division of Family Practice and other community partners. The Vernon UPCC provides urgent primary care which addresses healthcare needs that must be met within 12-24 hours within a primary care setting. In addition, The Vernon UPCC attaches patients for ongoing, longitudinal primary care, with a focus on marginalized and underserved members of the community. Specifically, these may include: Aboriginal, homeless seniors requiring palliative care, refugees, migrant workers, the transgender population, those with compromised executive functioning (Brain Injury/FASD), those involved with the justice system, (RCMP, Probation, Forensics, Parole, Incarceration), the uninsured (those living in poverty, the socially isolated with limited social supports, those with significant mental illness and substance use disorders and/or behavioral challenges), those with complex psychosocial needs, the financially challenged with economic barriers who are unemployed, homeless or marginally housed. Interior Health is seeking dynamic Nurse Practitioners to join our team at the Vernon Urgent and Primary Care Centre. The ideal candidate combines current or recent experience in primary care along with urgent or emergent care, with a clear understanding of the role and importance of foundational primary care, and is able to integrate the two. We actively foster a positive, respectful, and supportive working environment, and we are looking for someone who is positive, energetic, and empathic with strong communication skills and natural attention to detail. The UPCC offers work-life balance with competitive wages, within a group services contract providing flexible hours and days and the contract group self schedules. You’ll work together to ensure that the patient’s urgent needs are met in the most effective way possible. Some Typical Duties and Responsibilities Include: • Diagnoses and treats previously undiagnosed patients/clients/residents for undifferentiated diseases, disorders, and conditions within the Nurse Practitioner’s scope of practice; writes orders for treatment and medications; and provides first-line care in emergencies. • Monitors ongoing care, orders appropriate screening diagnostic investigations, interprets reports of investigations, and analyzes information to monitor progress and plan treatment. • Establishes priorities for management of health, diseases, disorders, and conditions; provides follow-up treatment; communicates with patients/clients/residents and families about health findings, diagnoses, priorities, outcomes, and prognoses; supports and counsels patients/clients/residents in their responses to diseases, disorders, and conditions. • Collaborates and consults with physicians or other healthcare and social service providers as appropriate to assess and diagnose patient/client/resident status. Develops and implements treatment plans. May admit and discharge patients/clients/residents to facilities according to organizational policies. • Prescribes drugs within the statutory and regulatory standards, limits, and conditions for Nurse Practitioners and within applicable employer policies and procedures. • May participates in research contributing to improved patient/client/resident care and advances in nursing, health policy development, and population health. • Maintains population health focus by implementing screening and health promotion activities for populations at risk. • Participates in ongoing education and skill development to maintain safe and evidenced based practice. May also mentor and support NP and/or medical students at the site when required. • Performs other related duties as required.
