Rutland UPCC is seeking a family physician to provide primary care services within its Longitudinal Care Clinic. This interdisciplinary clinic is comprised of on-site Physicians, NP's, RN's and Social Workers who utilize a team-based care model. The patients who are attached to this clinic have primary care needs that include but are not limited to:
- Chronic disease management
- Substance use disorders
- HIV primary care
- Gender affirming care
- Refugee care
- Full spectrum family practice
This is a unique opportunity for an incoming physician to take over a small, stable pre-existing patient panel as well as grow the panel size as they see fit, with support from an incredible allied health team.
