Role and Facility Information
Foundry Surrey is an integrated youth services (IYS) initiative for youth ages 12-24 and their families. Centrally located near public transit, universities and schools, Foundry Surrey will be the largest Foundry opened to date. Foundry Surrey aims to remove barriers and improve care pathways for youth by supporting co-location and integrating existing resources and services for five core streams: physical and sexual health, mental health, substance use, peer support and social services. With over 30 community partnerships and programs integrating into Surrey-Foundry, the Family Physician will work alongside an interdisciplinary team of service providers to meet the primary care needs of young people through a holistic, youth-centric and trauma-informed lens.
Foundry Surrey Highlights:
Youth-Oriented Environment: Long-standing reputation (Foundry) for compassionate, patient-centered care.
EMR System: We utilize the Myle EMR system to streamline patient record management and support.
Facilities and Space:
Examining Rooms: Beautifully designed exam rooms in a newly built facility.
Telemedicine Options: FPs will be offered telemedicine options at the beginning (e.g., phone call appointments) to expand this to virtual care as we become more established.
Hours of Operation: Once fully opened, are Monday-Saturday from 9:00 am-7:00 pm, with the last appointments occurring at 6:00. Schedule to be discussed individually to align with the FP contract requirements
Compensation and Financial Arrangement:
PCN/APP FP contract. In addition to the FP contract, the ministry will provide Foundry Surrey with the overhead.
