Nurse Practitioner Opportunity – Join the Primary Care Network in Chilliwack, Hope, or Agassiz Bring your clinical expertise and passion for primary care to a rewarding Nurse Practitioner (NP) role within the Primary Care Network (PCN) in Chilliwack, Hope, or Agassiz. The PCN, a collaboration between the Ministry of Health, Fraser Health Authority, and the Chilliwack Division of Family Practice, is transforming primary health care by building an integrated, community-based system that supports both wellness and patient care. Why Join Us? We are seeking Nurse Practitioners interested in working in a community-based practice who will work alongside other primary care providers in a collaborative, team-based model. As an NP in our PCN, you will have access to a multidisciplinary support team, including: • Counsellors • Social workers • Primary care clinical pharmacists • Chronic disease team (nursing, physiotherapist, dietitian, and occupational therapist) • Traditional wellness mentors What We Offer: • Full-time or part-time contract opportunities available • Competitive remuneration: $150,000–$160,000, based on experience • Full voting rights within the Chilliwack Division of Family Practice • Broad scope of practice, including vulnerable population outreach & longitudinal primary care • Local NP peer support network & Division-hosted social, wellness, and educational events • Opportunities for continued education, teaching, and mentoring NP students What You Bring: • Current provisional or practicing registration as a Nurse Practitioner with the BC College of Nurses and Midwives (BCCNM) • Relevant clinical nursing experience
